VARICOCELE EMBOLIZATION is a type of medical procedure. It diverts blood away from an enlarged vein in your sc***um. Such a vein is called a varicocele. It can cause pain, infertility, and swelling. An interventional radiologist uses a coil or special agents to block (or embolize) the vein. This can help improve symptoms.
A case of a 34/M with scrotal pain and infertility consulted with Interventional Radiology service for his condition of ultrasound proven LEFT VARICOCELE.
Procedure done as outpatient with local anesthesia. Admitted 0800H, varicocele embolization at 1100H, discharged at 1400H.
Patient tolerated the procedure well.
Patient was advised for inguinoscrotal ultrasound at 3 months and seminalysis at 6 months.

38/Female patient with dysfunctional uterine bleeding and adenomyosis referred to Pinoy-IR service for Uterine Artery Embolization.
Uterine artery embolization (UAE) is an interventional radiological technique to occlude the arterial supply to the uterus and is performed for various reasons.
Indications includes:
-intramural fibroids
-menorrhagia
-pelvic pain and pressure symptoms
-bladder outlet obstruction and hydronephrosis due to ureteric compression
-fibroids with otherwise undiagnosed infertility
as a preoperative measure for large fibroids
-dysfunctional uterine bleeding
-adenomyosis
-other less common indications include
uterine artery pseudoaneurysm(s)
-uterine AVM: trauma, post curettage
if active extravasation of contrast is detected during angiography for another reason (post-trauma)

38/Female referred to Pinoy-IR service due to bleeding renal mass (Angiomyolipoma)
Angiomyolipoma is a benign neoplasm of the kidney, and is the most common renal tumour associated with haemorrhage. Risk of bleeding increases with the size of the angiomyolipoma.
Risk of bleeding increases with the size of the angiomyolipoma. There is a 13% risk of bleeding if the lesion is < 4 cm and a 51% risk if > 4 cm.

INTERNALIZATION OF PTBD CATHETER
PTBD is an invasive and effective therapeutic method of relieving benign or malignant biliary obstruction and may be life saving if the patient is septic.
The drainage is achieved by inserting a plastic tube, called a catheter, through a tiny incision of the skin into the obstructed bile duct.
The cholangiogram performed in the same setting also helps to define the anatomy and identify pathology/abnormality, which are important for planning treatment.
Technical successful rate ranges from 90-95% in dilated system to 70% in non-dilated systems.
Subsequent internalization (either internal/external drainage or internal drainage by stenting on another day, usually days or weeks after the initial PTBD) of the PTBD facilitates internal drainage of bile, which is useful for digestion and therefore reduces the loss of fluid and electrolytes. If the PTBD is entirely internalized by stenting, this may eliminate the inconvenience of carrying an external tube on the abdominal wall.
