MGA KAILANGANG DALHIN:
1. Student/Recipient
1.1. Valid/School ID (original)
1.2. Photocopy of valid/school ID with 3 specimens of signature. Pakisulat din ng malinaw sa upper left corner ng photocopy ang inyong school at payroll number.

2. Representative (first degree including siblings only)
2.1. Representative’s Valid ID (original)
2.2. Photocopy of representative’s valid ID with 3 specimens of signature.
2.3. Photocopy of recipient’s valid/school ID with 3 specimens of signature. Pakisulat din ng malinaw sa upper left corner ng photocopy ang inyong school at payroll number.
2.4. Authorization letter from the program’s recipient (student)

EDUCATIONAL ASSISTANCE & SCHOLARSHIP PROGRAM PAYOUT
Handog ng Lokal na Pamahalaan ng Iba sa pamumuno ni Mayor Irenea Maniquiz-Binan!

Para sa lahat ng mga mag-aaral na Ibanian na nag-aaral sa loob at labas ng ating bayan, gaganapin ang ating payout sa darating na January 21, 2024 sa Provincial Gymnasium, Zone 6, Iba, Zambales.

MGA KAILANGAN TANDAAN:
1. Bago magtungo sa nasabing payout, siguraduhin na nakasali muna ang inyong pangalan sa listahan ng mga benepisyaryo (abangan ang link sa page na ito). Makikita ang schedule/oras sa payroll na i-uupload sa mga susunod na post.

2. MAGHANDA ng 1 photocopy ng inyong ID na may tatlong pirma. Isulat din ang inyong payroll/list number sa nasabing dokumento/photocopy.

3. Para sa mga representative ng mga mag-aaral na hindi makakapunta, kayo ay dapat nasa 1st degree na kamag-anak ng mag-aaral (magulang, asawa, o kapatid ay pinapayagan). Siguraduhin na may dala kayong 1 photocopy at original copy ng inyong ID at 1 photocopy ng ID ng mag-aaral. Huwag kalimutang lagyan ng tatlong pirma ang mga photocopy. Ipakita rin ang authorization letter.
