I. IN-PATIENT SERVICES AND SPECIAL PROCEDURES
A. Hospitalization Benefits
- room and board
- professional services of an accredited doctor or specialist
- use of medical or surgical facilities and equipment, operating and recovery room, intensive care unit (ICU), dialysis and chemotherapy equipment
- laboratory tests, X-ray, and other prescribed diagnostic or therapeutic procedures
- prescribed drugs and medication used in hospital
- dressing, plaster cast and other medical supplies
- administration of anesthesia or oxygen
- transfusion of hospital-provided whole blood or plasma

B. Special Procedures
- x-ray
- ultrasound
- basic mammography
- treadmill test and / or 2D Echocardiography

2. OUTPATIENT PRIVILEGES
-up to 15 Consults per year
-up to 12 pre/post natal consultation per pregnancy
- treatment of minor illnesses or injuries
- minor surgeries
-EENT ( eyes, ears, nose and throat )

B. Annual Physical Examination ( 2X ANNUAL CHECK UP)
- taking of medical history
- medical examination
- chest X-ray ( posterior/anterior )
- CBC
- stool and urine examination
- uric acid test
- FBS
-ECG (30y/o and above)
- Paper Smear ( 30y/o and above)

C. PREVENTIVE HEALTH CARE

D. DENTAL CARE
- Up to six (6) consultation per year
- Semi Annual oral prophylaxis ( 3 mos. After my effectivity date)
- Tooth extraction but excluding surgery
- Temporary filling or re-cementation
- Treatment of oral lesions, wounds and burns
