Do your migraines, persistent headaches and recurring neck or low‑back pain feel like separate problems — but never fully resolve? At Peek Practice we view them through a single, evidence‑based lens: some people have a more sensitive nervous system (nociplastic pain) that amplifies normal signals into ongoing pain.
As New Zealand’s only Board‑Certified Chiropractic Orthopaedist, I assess both structural drivers and central nervous system sensitivity so management addresses the real cause — not just the most painful spot. That can mean targeted structural care alongside graded activity, education and referral where needed.
If previous treatments haven’t helped, consider an expert assessment that clarifies whether multiple pains are expressions of one underlying process. Book a consultation at our Grey Lynn clinic or visit the link to read more.

Pain is not a signal your body sends. It is an output — generated when your nervous system calculates that threat outweighs safety. A persons psychological state (stress and emotional factors), along with their genetic predisposition and their social and belief systems also play a role in what determines their unique pain experience.
Pain as a Brain Output | Chiropractic Orthopaedist Auckland Nigel Peek, MSc Chiro (SA) | DIANM, Chiropractic Orthopaedist | PGDip HealSc Pain & Pain Management – Dist. Pain is an output. Not and Input. A protective output — generated when your nervous system calculates that threat outweighs safety. Pain is not a damage report per se, it is a complex interaction of factors based on perceived threat in addition to damage factors (see prior blog post).
That's why two people with similar MRI findings can have completely different pain experiences. The scan shows tissue, in other words a scan is a damage report, not a pain report. It doesn't show context, history, fear, or what happened last Tuesday.
This is the neuromatrix model — decades of neuroscience, now shaping how the best clinicians approach persistent pain.
It doesn't mean your pain isn't real. It means the story is more about you, the individual and what is happening in your life around your pain experience. This opens other avenues for treatment when applied on this basis.

Pain is the brain’s protective output — not always a direct report of tissue damage. Discover why you can have severe pain with no clear structural cause and vice versa. Read our deep-dive on current pain neuroscience and what it means for treatment and recovery: https://wix.to/tR2biIt
Key takeaways: pain is an experience, tissues and nerves interact with context, and modern definitions reflect that complexity. Share if this changed the way you think about pain.

Pain Is Not a Damage Report — What Current Pain Neuroscience Actually Says The short answerContemporary pain neuroscience tells is that pain is the brain's protective output — not a direct readout of tissue damage. The two are related, but they are not the same thing. You can have severe structural damage with no pain. You can have severe, disabling pain with no identifi...

Clear diagnosis matters. Lumbar discogenic pain often causes deep, central low‑back pain that worsens with flexion and sitting; radicular (referred) pain follows a nerve pattern down the leg with sharp, shooting symptoms and may show neurological signs on exam. At Peek Practice in Grey Lynn, Dr Nigel Peek — our internationally board certified chiropractor and pain management expert — uses evidence-based assessment: focused history, targeted neurological and orthopaedic examination and selective imaging (when indicated)*. Accurate diagnosis directs and determines the appropriateness of any treatment plan. At Peek Practice we present you with options that aim to get you moving as soon as possible. We do not just "crack backs" we use a range of treatment methods in accordance with your preferences. Our approach will likely include some or a mix of hands on treatment, education, targeted rehabilitation, and/or appropriate specialist referral. Sciatica can be misdiagnosed — and a second opinion can change everything. I am a Board‑Certified Chiropractic Orthopaedist at Peek Practice in Grey Lynn. We often see patients labelled with "sciatica” whose pain is actually driven by mechanical joint pain from the lower spine or hip. Sometimes these issues can be solved with a relatively simple approach that does not involve injections or surgery. If, however, your pain is consequential to nerve root compression with associated loss of function we can assist you with the right advice and appropriate timely referral. Our focused spinal assessment will help triage you into the right approach from the start. If you’re in Auckland and sceptical of trend‑style care, come for a measured, evidence‑based second opinion at 17b Pollen Street, Grey Lynn.
