CBCT Radiographic Interpretation of Maxillary Canine (UR3), impacted palatally.
Findings:
• The UR3 is visualized palatal to the maxillary arch.
• The crown of UR3 is oriented medially.
• Proximity and contact with the roots of the maxillary right central (UR1) and lateral incisors (UR2) with loss of lamina dura without external root resorption.
• Detailed color-coded segmentation of the maxillary dentition clearly shows the palatal impaction of UR3.
Orthodontic-Surgical Consideration: Surgical exposure and orthodontic traction of UR3 should be cautiously planned, prioritizing preservation of UR2/UR1.

Impacted maxillary third molar
Findings:
1. Tooth position and pathology:
• The left maxillary third molar is horizontally impacted, with the crown oriented mesio-superiorly.
• The crown is encompassed by a well-defined, expansile radiolucent lesion, consistent in morphology with a dentigerous cyst.
2. Cystic Lesion and Bone Involvement:
• The lesion is extensive, with resorption of the maxillary sinus floor, the buccal cortical plate, and the posterior wall of the maxilla on the affected side.
• There is also erosion of the lateral wall of the nasal cavity, indicating advanced and destructive expansion.
3. Maxillary Sinus and Nasal Cavity:
• The lesion causes significant invasion into the maxillary sinus cavity and buccal space, with loss of cortical boundary and apparent communication.
• The adjacent sinus shows signs of mucosal thickening, suggesting early inflammatory response.
4. Effect on Adjacent Tooth:
• The distobuccal root of the adjacent second molar shows external root resorption, likely due to pressure from the expanding cystic lesion.
Conclusion:
Horizontally impacted left maxillary third molar associated with an extensive, well-defined dentigerous cystic lesion, causing:
Recommendation:
Urgent referral for surgical evaluation and histopathological confirmation is advised due to aggressive bone destruction and risk of further maxillofacial compromise.

CBCT images showed the anatomical relationship between the mandibular right third molar (LR8) and the inferior alveolar canal (IAC), highlighting the root pressing the canal. Multiplanar reconstructions (axial, coronal, and sagittal) confirm intimate spatial proximity. The 3D volumetric renderings further delineate the curvature and position of the root apex relative to the IAC. White arrowheads mark critical contact points, and red lines trace the canal's course, emphasizing the necessity for surgical caution and preoperative planning.
