As sustainability becomes a growing concern in healthcare, a recent scientific review compares the environmental impact of traditional fixed appliances and clear aligner therapy.
๐ The findings are thought-provoking: both treatment options have an environmental footprint, but in different ways
- ๐๐ฅ๐๐๐ซ ๐๐ฅ๐ข๐ ๐ง๐๐ซ๐ฌ tend to generate more single-use plastic waste
- ๐
๐ข๐ฑ๐๐ ๐๐ฉ๐ฉ๐ฅ๐ข๐๐ง๐๐๐ฌ require energy-intensive manufacturing processes but involve more recyclable materials
๐ก So, can we really call orthodontics โsustainableโ today? Should environmental impact become a key factor in treatment choice?

Is Indirect Bonding more precise than Direct Bonding? What does the literature say?
Bracket placement accuracy plays a critical role in orthodontic treatment outcomes. Even small positioning errors can affect torque expression, rotations, and the overall finishing phase.
Several studies have compared Direct Bonding and Indirect Bonding to evaluate their accuracy.
๐ A systematic review published in 2023 concluded that indirect bonding tends to be more predictable and precise, reducing bracket placement errors compared with direct bonding.(Source: Patano et al., European Review for Medical and Pharmacological Sciences, 2023)
๐ A literature review including 21 studies also reported that indirect bonding generally provides greater bracket placement accuracy, while maintaining similar bond failure rates and overall treatment effectiveness.(Source: Soleiman, Archive of Orofacial Data Science, 2025)
๐ Earlier comparative studies have also shown that indirect bonding may provide better control of bracket height, an important parameter for proper expression of orthodontic prescriptions.(Source: American Journal of Orthodontics and Dentofacial Orthopedics)
Why does this matter clinically?
Even deviations of fractions of a millimeter or a few degrees in bracket placement can lead to:โข unwanted rotationsโข inaccurate torque expressionโข longer finishing phases and bracket repositioning
By positioning brackets digitally, indirect bonding allows clinicians to plan height, angulation, and rotation in a more controlled environment before transferring them intra-orally.
With the rise of digital indirect bonding workflows (CAD/CAM, printed trays), orthodontists now have tools that can further improve the reproducibility and precision of bracket placement.
โก Better bracket positioning from the start often means more predictable treatment outcomes and fewer adjustments later in treatment.
Years after treatment, patients return with lower incisor crowding and ask: "Why are my teeth moving again?โ.
The better question might be: Did they ever truly stop moving?
1๏ธโฃ Teeth are not fixed in stone
Teeth are supported by living bone and periodontal ligaments that continuously remodel throughout life. Orthodontic movement relies on this adaptability โ and that adaptability never fully stops.
๐ Source: Long-term orthodontic stability research published in Applied Sciences (MDPI, 2023).
2๏ธโฃ Soft tissue โmemoryโ exists
Supracrestal and periodontal fibers can retain a memory of the original tooth position. Without adequate retention, relapse may occur over time.
๐ Source: Orthodontic relapse studies in Applied Sciences (MDPI, 2023).
3๏ธโฃ Aging naturally changes alignment
Even untreated patients develop lower incisor crowding with age. Craniofacial growth, arch length reduction, and soft tissue changes contribute to this phenomenon.
๐ Source: Clinical reviews on post-treatment stability and age-related crowding.
4๏ธโฃ Retention is not optional
Multiple long-term studies confirm that stability depends on prolonged โ often lifelong โ retention.
๐ Source: Evidence-based orthodontic retention guidelines.
๐ Takeaway
Orthodontic treatment creates alignment. Retention preserves it. Long-term stability isnโt passive โ itโs biological, mechanical, and behavioral.
