63 old male suffering from dysphagia for 2 months went to ent doctor who sayed its quinsy in right side and made incision under local anesthesia no improvement happened at all ,
Came to my clinic after 2 wks after incision and the edges are still open
Ultrasound were suspicious ,CT with contrast was done ,
Also suspicious finally MRI with contrast
Was Ordered ,excisional biopsy was the recommendation
And full lab was ordered to perform surgery .
DD??
Benign or malignant??
Risk in this surgery??

Differential diagnosis??
The left maxillary sinus !!
70 y old female blood sugar 600 for more than one month,presented with retro pharyngeal abscess opining to posterior pharyngeal wall and fever .
Hb 5 INR 1.6 ,
admission to ICU , RBCs and plasma was introduced to her cleaning was done and widening of the opening achieved intra-operative nasogastric tube was introduced under GA after three days she developed mild neck pain left side
CT neck will be taken tonight .
Rather than that hb became 12
Fever subside

This is my cousin CT
Suffering from severe headache and drop eye led on left eye as you can see .
I will wait for antibiotic and steroid to give its effect
And after that I think navigation system is the best choice
With Allah help we can see him cured after few days .(fungal sinusitis ) is my diagnosis 🤔

Uncontrolled diabetes
42 year
Was severely infected from dental origin
The incision must kept open to drain all the infection
I didn’t need to do tracheostomy
He was easily intubated
Female pt complain from left frontal and orbital pain

73 y old lady complaining from left ear pain from one month she went to several ear doctors they treat her as otitis media
With no response
After CT i think the story is clear
