Retraction and Delamination as Surgical Challenges in a Traumatic Cuff Tear By Dr. Prakash Ayyadurai
🔹 Case Summary
Mr. X, 48-year-old male farmer, right-hand dominant, presented with persistent right shoulder pain and weakness following a road traffic accident one month earlier.
🔹 Clinical Findings
• Positive Drop Arm test
• Good internal rotation
• Weak external rotation
🔹 MRI Findings
• Medially retracted full-thickness supraspinatus tear
• Horizontal split / delamination component
• Thinning of the superior cuff
• Preserved subscapularis
• Associated traumatic biceps tendinosis
Traumatic rotator cuff tears with significant retraction and delamination pose unique surgical challenges. Tissue quality, tendon mobility, and restoration of cuff balance become critical factors influencing repair strategy and postoperative recovery.
Early diagnosis and timely intervention are essential, particularly in active individuals whose livelihood depends on upper-limb function.
