: 🇨🇭🇬🇧 From a nightmare to a healthy smile, a highly esthetic implant-supported Overdenture
▪ On a Saturday, a 40 years old woman complained from a broken provisional bridge on 4 maxillary implants, unable to eat, speak properly or even smile again. Implants & prosthetic work on top were done abroad, outside of Switzerland.
▪ Antagonist teeth suffering from moderate periodontal disease have been stabilized alongside the new prosthetic work, and all teeth & implants are currently under strict periodontal maintenance (Mombelli et al., 2011).
▪ With respect to her limited financial resources, the treatment plan & estimated costs were accepted.
After few visits, a new maxillary implant-supported Overdenture was created: more aesthetic, fracture resistant and facilitating cleaning around maxillary implants, compared to the previous provisional acrylic bridge.
▪ Patient was happy to smile and eat again with confidence, having a natural, aesthetic and functional smile (Guédat, Nagy, Schimmel, Müller & Srinivasan, 2018).

: 🇨🇭🇫🇷 D'un cauchemar à un sourire sain, une Overdenture implanto-portée hautement esthétique
▪ Un samedi, une femme de 40 ans s'est plainte d'un pont provisoire cassé sur 4 implants maxillaires, incapable de manger, de parler correctement ou même de sourire à nouveau. Les poses d'implants et prothèse par-dessus ont été réalisés à l'étranger, en dehors de la Suisse.
▪ Les dents antagonistes souffrant de maladie parodontale modérée ont été stabilisées parallèlement au nouveau travail prothétique, et toutes les dents et implants sont actuellement en maintien parodontal strict (Mombelli et al., 2011).
▪ En respectant ses ressources financières limitées, le plan de traitement établi & coûts estimés ont été acceptés.
Après peu de visites, une nouvelle prothèse maxillaire implanto-portée a été créée: plus esthétique, résistante aux fractures et facilitant le nettoyage autour des implants maxillaires, par rapport au pont provisoire en acrylique d’auparavant.
▪ La patiente était heureuse de sourire et de manger à nouveau en toute confiance, avec un sourire naturel, esthétique et fonctionnel (Guédat, Nagy, Schimmel, Müller & Srinivasan, 2018).

: 🇨🇭🇬🇧 A case of internal resorption on a lower central incisor
▪ At general check-up, a 55 years old woman complained from slight irritation at the area of her right lower central incisior. After clinical examination, a periapical X-ray was taken.
▪ As all neighboring teeth are healthy, internal resorption was clearly deep beneath the gum level, surrounding the neck of that lower right central incisor.
▪ Once the established treatment plan and estimated costs were accepted, the tooth was extracted atraumatically, then shortly after, a Straumann BLT 2.9 mm implant was inserted with a little simultaneous lateral bone augmentation (Donos et al., 2008).
▪ During implant healing and osseointegration, a provisional prosthesis was provided for the patient as she awaits her screw-retained crown and definitive restoration.
▪ Patient was happy to leave with a confident smile and be able to eat with no apparent missing tooth (Buser, Chappuis, Janner & Belser, 2019).

: 🇨🇭🇫🇷 Un cas de résorption interne sur une incisive centrale inférieure
▪ Lors d’un check-up général, une femme de 55 ans s'est plainte d'une légère irritation au niveau de son incisive centrale inférieure droite. Après examen clinique, une radiographie périapicale a été prise.
▪ Comme toutes les dents voisines sont saines, la résorption interne (ou granulome dentaire interne idiopathique) était clairement profonde sous le niveau de la gencive, entourant le col de cette incisive centrale inférieure droite.
▪ Une fois le plan de traitement établi et les coûts estimés acceptés, la dent a été extraite de manière atraumatique, puis peu de temps après, un implant Straumann BLT 2,9 mm a été posé avec une petite augmentation osseuse latérale simultanée (Donos et al., 2008).
▪ Lors de la cicatrisation implantaire et de l'ostéointégration, une prothèse amovible provisoire a été mise en bouche de la patiente en attendant sa couronne trans-vissée sur implant et sa restauration définitive.
▪ La patiente était contente de repartir avec le sourire et de pouvoir manger sans dent manquante (Buser, Chappuis, Janner & Belser, 2019).

▪ Plaque accumulation at 1 mm height of kertainized tissues, transformed 6 weeks later to a deeper vestibule with a simple Free Gingival Graft restoring a much better height of attached kertainized gingiva of > 7 mm.
▪ Gingiva is rather firm, of pink color and much healthier; absence of pockets of > 3 mm in depth.
▪ Patient was happy to keep her teeth, be able to clean better (plaque control), diminish the sensitivity to cold and have a more confident smile (Matter and Cimasoni, 1976; Matter, 1980).

: 🇨🇭🇫🇷 Chirurgie mucogingivale pour restaurer l'esthétique et fonction autour des incisives inférieures
▪ Meilleure esthétique pour une femme de 44 ans.
▪ Accumulation de plaque à 1 mm de hauteur de tissus kertainisés, transformés 6 semaines plus t**d en un vestibule plus profond avec une simple Greffe Gingivale Libre rétablissant une bien meilleure hauteur de gencive attachée kératinisée de > 7 mm.
▪ La gencive est ferme, de couleur rose et beaucoup plus saine; absence de poches > 3 mm de profondeur.
▪ La patiente était heureuse de garder ses dents, de pouvoir mieux nettoyer (meilleur contrôle de la plaque dentaire), de diminuer la sensibilité au froid et de pouvoir sourire avec confiance (Matter et Cimasoni, 1976; Matter, 1980).

: 🇨🇭🇬🇧 The beauty of Periodontal & Orthodontic (Perio-Ortho) treatments and synergy
▪ The aesthetics are much better for a man of 29 years old.
▪ Black triangles due to gingival retraction and absence of inflammation can be partially masked/closed with composite resin.
▪ Gingiva is rather firm, of pink color and much healthier; absence of pockets of > 4mm in depth.
▪ Next step is Orthodontic refinement: removal of cleats, bonding palatal and lingual retainers, as well as a superior mouth-guard for support of posterior teeth
▪ Patient was happy to realize all his desires: preserving all his teeth, diminishing the sensitivity to cold and mobility of anterior ones, as well as being able to eat with ease with all his teeth.
▪ After almost 4 years of treatment, the result and General Prognosis are good. The Local Prognosis is also good, as long as the patient remains motivated to maintain an excellent oral hygiene, does not smoke and comes regularly to his maintenance recall visits (Mombelli et al., 2011).

: 🇨🇭🇫🇷 La beauté des traitements Parodontaux & Orthodontiques (Paro-Ortho) ainsi que leur synergie
▪ L'esthétique est bien meilleure pour un homme de 29 ans.
▪ Les triangles noirs dus à la rétraction gingivale et à l'absence d'inflammation peuvent être partiellement masqués/fermés avec de la résine composite.
▪ La gencive est plutôt ferme, de couleur rose et beaucoup plus saine; absence de poches > 4 mm de profondeur.
▪ La prochaine étape est le raffinement orthodontique: retrait des taquets, adhésion des contentions palatines et linguales, ainsi qu'une gouttière supérieure pour le soutien des dents postérieures
▪ Le patient était heureux de réaliser tous ses désirs: préserver toutes ses dents, diminuer la sensibilité au froid et la mobilité des antérieures, ainsi que pouvoir manger aisément avec toutes ses dents.
▪ Après presque 4 ans de traitement, les résultats et le pronostic général sont bons. Le Pronostic Local est également bon, tant que le patient reste motivé à maintenir une excellente hygiène bucco-dentaire, ne fume pas et se présente régulièrement à ses visites de Suivi/Maintien Parodontal (Mombelli et al., 2011).
