BY APPOINTMENT ONLY
During this time, we are continuing to adhere to strict guidelines
to ensure that all areas of our office are disinfected at all times

To prepare for your next appointment, we respectfully request the following from our patients:
- All patients will be prescreened to ensure they are well and
symptom-free.
- Wear a face mask to your appointment.
- You will be required to sanitize your hands before proceeding through the office.
- Limit the number of attending people to the appointment to the patient
only and a caregiver (if needed).
- Limit the number of family members booked at a time.
- Anticipate longer appointment times to ensure social distancing and
sanitization practices.

Your safety and quality of care are important to us, so we kindly ask for
your patience and understanding.

June 15th, 2019 will be the last Saturday that we will be open. Our summer hours start from Monday June 17, - Sunday September 1, 2019. Summer hours are Mon., Tues., Wed. 10:00am - 6:00pm and Thurs 10:00am - 1:00pm.
