Almost two years after rhinoplasty. The tip and nostrils were narrowed , the bridge was augmented with rib cartilage . Our goal here mostly to try and create definition and narrowing from the front view . In patients with very thick skin this accomplished by augmenting the underlying structure .

Great day in the OR . Three noses , all different and amazing patients . Case number one was a challenging crooked nose with a small bump and tip refinement. Case 2 was more of an over projected nose with a bulbous tip . Case 3 was a wide nose , significant narrowing of the bridge using piezo was performed and refinement of the tip. We hope they their results.

On the table results after “ultrasonic”rhinoplasty. This is a good case to show how much the bridge can narrowed and reshaped. This is a less common concern among our patients but can have a nice effect on facial balance. Ultrasonic rhinoplasty refers to using the piezotome , a tool we use to reshape and cut the nasal bones. I find it very useful in cases like these. It’s not really a different way of doing rhinoplasty but just one of the tools we can use to optimize your result.

One year following rhinoplasty in a patient with very thick skin. The tip was lifted and refined and the bridge was reduced . The final photo shows results at 3 months and one year after surgery. The tip continues to refine for a few years after surgery in patients with thick skin.

Four months results in a patient with very thick skin. The remaining swelling in the tip will gradually go down over the next year. The tip was refined and projected and the nostrils were narrowed ( alar base reduction). When a patient has thick skin we can still get great results but it takes longer to see the final outcome. Nasal taping, skin care to reduce oil, cortisone injections and sometimes using oral retinoids like Accutane can help.
