Ice dams and icicles form when the snow melts, runs down your roof and refreezes near the edge. There follows a freeze thaw cycle that works the ice up under the shingles like the movement of a glacier. This only occurs when part of your roof warms to above 0 degrees C, warm enough to melt the snow, while the roof edge remains below freezing. This scenario is often the result of a warm attic.
Lack of ventilation and low insulation also the frost built on sheathings and grow mold on them.
