# পোড়ার প্রাথমিক চিকিৎসার চ্যালেঞ্জ

পোড়ার প্রাথমিক চিকিৎসার চ্যালেঞ্জগুলির মধ্যে রয়েছে পোড়া স্থানের দ্রুত এবং যথাযথ ঠান্ডা করা নিশ্চিত করা, সংক্রমণ প্রতিরোধ করা, ব্যথা কমানো।

এখনও অনেকে পোড়ার সাথে সাথেই ভুল বিশ্বাস থেকে অনেক ক্ষতিকর উপকরণ ব্যবহার করেন । এর মধ্যে রয়েছে ডিমের সাদা অংশ , টুথপেস্ট, কেরোসিন, নারকেল তেল, পাতা, গোবর এবং মাটি।

কারো বদ নজর যেন না লাগে সেজন‍্য অনেকে চুল বেঁধে দেয়। এবার দেখলাম পিয়াজ বেঁধে দিয়েছে নজর না লাগার জন‍্য।

পোড়াস্হানে এসব উপকরন ব‍্যবহার ক্ষতিকারক হতে পারে। কারন পোড়াস্হানে ত্বকের প্রতিরক্ষামূলক বাধা ব্যাহত হয় । তাই এগুলোর ব‍্যবহার সংক্রমণের ঝুঁকিতে ফেলে এবং superficial বার্ন deep বার্ন এ রূপান্তরিত হতে পারে যা পরবর্তীতে সার্জারির প্রয়োজন হয়।

# পোড়ার জন্য প্রাথমিক চিকিৎসা

১. পোড়া প্রক্রিয়া বন্ধ করুন:
পোড়ার উৎস থেকে ব্যক্তিকে সরিয়ে ফেলুন (যেমন, তাপ)

২. পোড়া জায়গা ঠান্ডা করুন:

- বেশিরভাগ পোড়ার ক্ষেত্রে, জায়গাটি ১০-২০ মিনিটের জন্য
প্রবাহমান পানির ( running tape water)নিচে রাখুন।

- বরফ ব্যবহার করবেন না: বরফ বা বরফের পানি ব্যবহার
আরও ক্ষতি করতে পারে।

৩. পোড়া জায়গার কাছাকাছি জিনিসপত্র সরিয়ে ফেলুন:

- পোড়া জায়গায় আটকে না থাকা যেকোনো পোশাক বা
গয়না আলতো করে সরিয়ে ফেলুন।

- ত্বকের সাথে লেগে থাকা জিনিসপত্র জোর করে সরাবেন না,
কারণ এতে আরও ক্ষতি হতে পারে।

৪. পোড়া জায়গাটি সুরক্ষিত রাখুন:

- ঠান্ডা হওয়ার পর, পোড়া জায়গাটি একটি পরিষ্কার, নন-
স্টিক ড্রেসিং দিয়ে ঢেকে দিন। I

এক্ষেত্রে হাসপাতালে জীবাণুমুক্ত পদ্ধতিতে ড্রেসিং করা
ভালো।

Contracture development after burn or trauma is common and many of them are preventable if early measures are taken.
Treating surgeon can follow few simple steps to reduce development of contracture-
Appropriate splinting during wound management
and early days after healing Avoiding skin graft around joints Practicing early range of motion exercises to
maintain joint flexibility and preventing stiffness.

This gentleman was advised to go for amputation. After having consultation with him with possible negative outcomes, we planned to go for reconstruction. The first challenge was to cover such a complex wound. After adequate coverage by plastic surgery team, the orthopedic team did their best to get the final result.
Final result: Patient is walking on his feet!!!

Zone 2 flexor tendon injuries:

Zone 2 flexor tendon injuries are historically termed as “no man’s land”as there are difficulties during treatment, chance of adhesion in the postoperative period resulting in poor clinical outcome. Adequate repair is essential to overcome these problems by ensuring early mobilization in post op. Appropriate repair can regain the lost hand cascade as well.

Lower limb trauma: Free tissue Transfer

Lower limb injuries due to motor vehicle accidents are the major bulk of patients attending in all trauma centres. Open wounds following these injuries need management by multidisciplinary approach. After bone stabilization, management of soft tissue becomes a challenging task in many situations. Free tissue transfer( free flap) is a versatile technique for coverage of these complex wounds which can help early wound healing as well as to reduce complications.

This patient had soft tissue defect involving dorsum of foot exposing tendons and weight bearing heel area. An ALT free flap was planned according to template of the defect which adequately covered both the wounds.

হাত পুনর্গঠন

মেশিন বা সড়ক দুর্ঘটনাজনিত কারণে অনেক সময় হাত আঘাতপ্রাপ্ত হলে হাতের চামড়া,মাংস এমনকি হাড় ক্ষতিগ্রস্ত হয়। এই ক্ষতিগ্রস্ত অংশ সঠিক সময়ে উপযুক্ত পদ্দ্বতিতে পুনর্গঠন করতে না পারলে অনেক সময় ক্ষতিগ্রস্ত অংশ কেটে ফেলে দিতে হয়।

এই পুনর্গঠনের জন্য অনেক ক্ষেত্রেই ফ্ল্যাপ সার্জারির প্রয়োজন হয়। অধিকাংশ সময়েই পেটের অংশ হতে ফ্ল্যাপ নিয়ে ৩ সপ্তাহের জন্য হাত পেটের সাথে আটকিয়ে রাখতে হয়। হাতের উপরের অংশ থেকে ফ্ল্যাপ নিয়ে সার্জারি করা সম্ভব হলে রোগীর জন্য সেটা কিছুটা স্বস্তিদায়ক হয়।

Wound over tendoachilles area can occur following trauma or Indian toilet pan injury. Coverage with a durable flap is needed for good healing as well as to reduce further complications.

Perforator based propeller flaps are better choice for small and medium sized defects. In small wounds , coverage can be done without the need of skin graft in donor area.

বোটক্সঃ

বোটক্স এক ধরনের নিউরোমডুলেটর যা কিছু টার্গেটেড মাসলে ইনজেক্ট করা হয়। ফেসের মাসলগুলো রিপিটেড কন্ট্রাকশনের কারণে কিছু এক্সপ্রেশন লাইন এবং রিংকেলস তৈরি করে।

বোটক্স ইনজেক্ট করলে তা এই মাসলগুলাকে রিলাক্স করে এবং এদের মুভমেন্ট কমিয়ে দেয়, ফলশ্রুতিতে এক্সপ্রেশন লাইন ও রিংকেলস কম বুঝা যায়।

বোটক্স ইনজেকশনের পর ফলাফল পেতে কত সময় লাগে?

সাধরণত ইনজেকশনের ইফেক্ট পেতে ৩-৭ দিন সময় লাগে। কারো কারো ক্ষেত্রে ২ সপ্তাহও লাগতে পারে

বোটক্স কতদিন স্থায়ী হয়?

সাধারণত ৩-৬ মাস পর্যন্ত এর ইফেক্ট থাকে। যারা দীর্ঘ সময় এর ইফেক্ট ধরে রাখতে চান , তাদেরকে ৩-৫ মাস পরপর রিপিট করতে হয়।


বোটক্স ইনজেকশন নিরাপদ কি ?

বোটক্স FDA approved ট্রিটমেন্ট , তাই এটি নিরাপদ। তবে ইনজেক্ট করার সময় সতর্কতা অবলম্বন করতে হবে এবং কেবলমাত্র নির্ধারিত স্থানে বিশেষজ্ঞ চিকিৎসকের মাধ্যমে ইনজেক্ট করতে হবে।

এর কোনো পার্শ্ব-প্রতিক্রিয়া আছে কি?

সঠিকভাবে পদ্ধতি অবলম্বন করলে সাধারণত তেমন কোনো সমস্যা হয় না। তবে অনেক সময় ইনজেকশন সাইট এ লাল হয়ে যাওয়া, একটু ফুলে যাওয়া বা সামান্য ব্যথা অনুভূত হতে পারে যা কয়েকদিনের মধ্যেই সেরে যায়।

ফেসের কয়েকটি স্থানে এক্সপ্রেশন লাইনগুলি বেশি পরিলক্ষিত হয় বলে সাধারণত এইসব স্থানে বোটক্স ইনজেকশন বেশি দেয়া হয়।
