It isn’t all ‘Push!’ and ‘Breathe’ during labour.
What we want you to know, is that during the pushing stage our pelvic floor needs to lengthen and release, not contract or brace. The baby passes through our levator hiatus (the U-shaped opening in the pelvic floor muscle), and this needs to be as relaxed as possible for this to happen.
Exhalation-based breathing during pushing (inhaling, then exhaling while bearing down) has been shown to reduce perineal trauma, help manage pain and can shorten the pushing stage significantly; while breath-holding while pushing - known as the Valsalva manoeuvre - increases downward pressure but doesn’t support pelvic floor release.*
Research has linked the Valsalva with greater perineal trauma and can affect how your baby copes with labour.*
At PhysioSista, our team can show you how to coordinate the release of your pelvic floor before birth, using real-time cues and a model to help you feel the difference. This is one of the things we can discuss and add to your plan during your initial assessment.

I’ve been pregnant three times.
And every pregnancy felt different in my body.
During my second pregnancy I remember feeling really frustrated when I was told:
“Your pelvis is fine. You’re just pregnant.”
Which… wasn’t especially helpful.
I wasn’t looking for reassurance that it was “normal”.
I was looking for relief.
Something practical.
A plan.
Something that would actually help me feel better.
Even as a physio myself, I had to push to find the right support.
It’s a reminder that not every health professional approaches care the same way. If something doesn’t feel right, or you’re not getting what you need, it’s okay to ask questions or seek another opinion.
Some discomfort may be a part of pregnancy.
But doing nothing shouldn’t be the only option.
You deserve support.
And a plan that helps your body feel and move better.

Pelvic pain isn’t always coming from the pelvic floor.
And “hip pain” isn’t always coming from the hip joint.
The pelvis, hips and lower back work closely together, which means symptoms in this area can sometimes feel confusing.
A lot of women tell us they have hip pain but point to their buttock or lower back. Others feel deep pelvic pain that seems to wrap around into the hip.
That’s why a good assessment looks at the whole system — the hip joint, pelvic floor, lower back and the way your body moves.
Our two newest physios, Mel and Krystal, bring extensive experience treating hips and backs, alongside their pelvic health training.
Together, it means we’re well placed to help work out what’s really contributing to symptoms, and guide you towards the right plan.
If something doesn’t feel right in your body, it’s worth getting it checked.

The 6-week GP check is important but it’s not a movement assessment.
If you want to return to exercise without guessing, compensating or hoping for the best, a proper postnatal physio screen changes everything.
