We spend so much time talking about egg quality, but not nearly enough time talking about s***m quality.

Male fertility contributes to around 50% of fertility challenges, which is why a semen analysis is one of the most valuable tests to do early.

The problem is that many people are told their results are “normal” without understanding what those numbers actually mean for fertility.

In clinic, I’m not just looking at whether a result falls within the lab reference range. I’m looking at whether it’s optimal for conception, embryo quality, miscarriage risk, pregnancy outcomes, and the long-term health of your future child.

What many people don’t realise is that s***m health doesn’t just influence whether fertilisation occurs. It can also influence embryo development, placental health, and pregnancy outcomes. Research has linked paternal health to complications such as recurrent miscarriage, preeclampsia, fetal growth restriction, and even severe morning sickness.

It’s beyond me that male fertility is still one of the most overlooked pieces of the infertility puzzle.

Around 40–50% of fertility challenges involve male factor infertility, yet the focus still often remains almost entirely on the woman.

What many people don’t realise is that s***m quality isn’t fixed. S***m are constantly being produced, with a full s***m development cycle taking approximately 70–90 days.

So factors like nutrition, stress, sleep, alcohol, smoking, environmental toxins, heat exposure, and overall health can significantly impact fertility outcomes.

Even when a semen analysis falls within the lab reference range, this doesn’t confirm results are optimal for conception.

And standard semen analysis doesn’t always tell the whole story. Factors such as s***m DNA fragmentation can affect embryo quality, implantation, and miscarriage risk, even when count, motility, and morphology appear relatively normal.

Silent endo is a very real and common driver for unexplained infertility.

Most people assume that endo only presents with pain, but it’s absolutely not always the case and in the years I have practiced as a naturopath, I have seen way too many women struggling to fall pregnant.. because they have with silent endo that no one has considered or investigated.

Beneath the surface, endometriosis creates an inflammatory environment in the pelvis.

And that inflammation can disrupt egg quality, impair ovulation, affect s***m function, reduce implantation, and alter the immune signalling needed to sustain pregnancy.

You can have cysts, inflammation, or partially blocked fallopian tubes all contributing to infertility or miscarriage without even knowing it.

If you’ve been told you have unexplained infertility or everything looks normal but you’re still not falling pregnant, it’s worth considering whether endometriosis could be part of the picture 🤎

“My periods are regular, so I must be ovulating”

This is one of the biggest fertility myths I hear in clinic.

Because while regular cycles can be a positive sign, they don’t always guarantee healthy ovulation.

You can bleed every month and still have cycles where no egg is released. You can also ovulate but not optimally.

Meaning:
• egg quality may be poor
• progesterone production may not be strong enough to support implantation
• the hormonal changes needed for a healthy fertile window may not be happening properly

This is also why period tracking apps can be misleading when trying to conceive. Most apps predict ovulation based on averages, not your body’s actual signs.

In clinic, I often see women ovulating much later than their app predicted, meaning they’re accidentally missing their fertile window altogether.

This is why I recommend tracking:
• basal body temperature
• cervical mucus
• luteal phase length
• properly timed progesterone

Because a regular bleed and a truly fertile cycle are not always the same thing 🩵

“Everything looks normal”

I can’t tell you how many couples I speak with in clinic who have been told this, while still not having answers for why they’re not conceiving.

Often they’ve had basic blood work done and been reassured that everything is “fine” because nothing flagged as abnormal.

But when it comes to fertility, there’s a big difference between a result being within range and it being truly supportive of conception.

Meaning in range iron may still be too low to properly support ovulation. Or a normal TSH doesn’t always reflect optimal thyroid function for fertility. And ovulation can be happening, but progesterone may show it’s not as strong or healthy as it needs to be.

Sometimes the issue isn’t obvious pathology. It’s subtle dysfunction being missed because nobody is looking at the full picture.

One of the most overlooked areas in fertility and recurrent miscarriage is the vaginal microbiome.

A recent 2026 study published in Fertility & Sterility found higher rates of both Mycoplasma and Ureaplasma in women with recurrent pregnancy loss.

The study also found:
– >95% cleared the infection after treatment
– and live birth rates were significantly higher in treated groups compared to culture-negative recurrent loss cases.

This is exactly why we order vaginal microbiome testing for our fertility clients, because time and time again we see women told they have “unexplained infertility” or recurrent miscarriage without ever being tested for these infections.

It’s important to know Ureaplasma is very common and doesn’t always require treatment, but in the context of fertility struggles, implantation issues, recurrent BV/thrush, or miscarriage, it’s definitely something worth investigating and clearing 🤎
