Responsible for performing tasks, designated by an RN case manager, for patients with complex medical and social needs to optimize patient health across the care continuum. Performs duties over the telephone and in person at variable locations, to meet the patient “where they are” such as patient homes, homeless shelters, hospital rooms etc. This is an opportunity to make a difference in the lives of some of the most vulnerable in our community. A passion for helping others and creative out-of-the-box problem solving skills are strongly encouraged.
ESSENTIAL FUNCTIONS
• Works directly with patients and their families to address social determinants of health including; transportation, housing, and food security.
• Makes home visits to patients at place of residence or other facility frequently to provide support and education.
• Works closely with RN Case Manager (supervisor) to ensure patient needs are met.
• Communicates with medical staff, social workers, and families at hospitals, clinics, telephonically, etc.
• Arranges or provides transportation for patients to provider appointments.
• Frequently attends medical and social service appointments with patients to eliminate barriers.
• Outreach and communication with community partners
• Daily census review follow-up phone calls with documentation
REQUIRED QUALIFICATIONS
• Valid WA driver’s license, and car insurance
• Must have own transportation and be willing to transport clients in your own vehicle (mileage reimbursed)
• Comfortable working with clients who may have mental illness and/or substance use disorder
• A demonstrated comfort level working with people & situations that can feel frustrating, unclear, & seemingly hopeless; Ability to work with a positive attitude in stressful situations.
• Ability to work independently with minimal direction
• Basic computer skills
PREFERRED QUALIFICATIONS
• Medical Assistant certification
• -OR-
• Nursing Assistant Certification
• Awareness of community resources

We are looking for a qualified Medical Office Manager to run our vaccine clinic. You will be responsible for day-to-day operation, budgeting and maintaining medical and staff records. As a Medical Office Manager, you should demonstrate excellent organizational skills and the ability to perform well in stressful situations. Prior knowledge of healthcare systems operations is highly desired. This role should be able to ensure steady workflows and uninterrupted service in our clinic.
Role and Responsibilities:
-Responsible for coordinating the ordering, delivery, receipt and storage of vaccine.
-Arranging scheduling software and ensuring that patients are registering and completing needed forms in preparation for the administration.
-Coordinating staff schedule for clinics
-Organization of office setting to accommodate flow of patients to maximize efficiency and reduce risk of infection.
-Responsible for ordering supplies needed for vaccine clinics.
-Arrange disposal of used syringes.
-Order and maintain medical safety equipment.
-Ensure completion and filing of documentation required for all adverse events.
-Arranging any travel and cold chain transportation of vaccine.
-Managing inventory of vaccine
-Overseeing staff on site at the vaccine clinic to promote efficiency, safety and outcomes.
-Ensure data entry is completed timely for vaccine registry.
-Greeting patients visiting the facility, answering any questions they may have and helping them fill out the required forms.
-Answering phone calls, creating appointments, directing the calls as required and handling all queries.
-Communicating with leadership, Nurse and vaccine clinic team about absences, scheduling conflicts, adverse events and other concerns.
-Maintaining a filing system for all patient documents and reports submitted.
-Answering emails and other electronic messages as required
-Coordinating the creation of invoices and billing. Ensuring the processing of insurance forms
-Managing vendors and contractors and arrange meetings as needed.
Required Qualifications and Experience:
-Proven work experience as a medical office manager
-Knowledge of accounting, data and administrative processes and principles
-Ability to handle medical records discreetly
-Excellent organizational and time-management skills
-Ability to multi-task and perform well in stressful situations
-Communication skills and a customer service orientation
-Knowledge of medical issues and terminology is desired.
-Complete trainings relevant to safe storage, handling, and administration of the vaccine.
-Valid driver’s license with the ability to travel locally as needed
-BS degree in Healthcare Administration, Business or relevant field is desired.
