In cranial remolding, the younger the patient starts treatment, the better.

The average starting age is 6 months of age, although 4 months old would be better. The most rapid period of growth is between birth and 8 months of age. Good growth exists between 8 and 12 months of age but, after 12 months of age growth slows significantly.

Depending on the age of the child at the start of treatment, a course of cranial remolding can last from 2- 6 months, sometimes longer.

Infants beginning treatment at age 3-4 months are often treated until they are 7-8 months of age to reduce the potential need for a second helmet to maintain correction.

At 8 months of age infants are usually rolling over, on their own, and sleeping on their stomachs. At this age their skulls are more calcified and they tend to be sitting up more. All these factors help reduce the likelihood of losing any correction that was gained during treatment.

A less common type of flat head syndrome is called BRACHYCEPHALY. Here we have a side by side depiction of a BRACHYCEPHALY head shape versus a normal head shape.

Signs of Brachycephaly:

-Viewing from the side of the head look for flattening in the back
of the head and an upwardly sloping forehead.

-A head presenting with a short frontal to occipital length.

-A wide width ear to ear.

-An upwardly sloping skull from forehead to occiput when viewed from the side.

-Often the result of supine positioning after birth.

There are multiple versions of “Flat Head Syndrome” that a child can be diagnosed with.

We've provided examples of a "Normal" head shape compared to a Plagiocephaly head shape. Keep in mind, you may not see these shapes exactly—variations ranging from mild to moderate to severe and even combinations of these shapes can occur.

Signs of Plagiocephaly:

-Head shape in the form of a parallelogram.

-Bulging of the forehead on the side as the posterior flattening.

-The ear on the flattened side is pushed forward toward the face.

Here is a side by side depiction of a SCAPHOCEPHALY head shape, another less common type of flat head syndrome, versus a normal head shape.

Signs of Scaphocephaly:

-Presents with a long frontal to occipital length.

-Narrow parietal width.

-Prominent occiput and broad forehead.

-Must rule out Craniosynostosis by CT scan.

The orthosis will facilitate midline head positioning while the infant is supine(on their back). Since the orthosis is a hard, symmetrically curved surface, the infant will be able to activate neck and trunk musculature to both sides. This will enhance their ability to roll from prone (belly) to supine (back) and back again, which are skills that provide a foundation for higher-level motor activities.

The orthosis will attempt to restore symmetry to the skull so that the child will be able to wear standard, protective head gear for bicycling, baseball, football and other activities as they mature.

Congenital Muscular Torticollis (CMT) or Wry Neck as it's called, is a disorder of the sternocleidomastoid (SCM) muscles of the neck. There are SCM muscles on each side of the neck. These muscles control rotation and tilting of the head.

In CMT, one of these muscles becomes shortened and although the exact etiology is not known, at least 40% of infants diagnosed with CMT have experienced a difficult birth. Some theorize in-utero positioning causes a contracture of one of the muscles and when the infant is placed on their back to sleep, this contracted muscle tilts and rotates the head.

Since the muscle is tight the infant yields to it and, given the infant's skull is soft, the mattress their head is lying on is hard and their head does not change position, the infant develops Plagiocephaly.

Sometimes known as “flat head syndrome,” plagiocephaly (pronounced play-jee-oh-sef-uh-lee) is a relatively common condition where an infant develops a flat spot on the back or backside of the head. Many factors can cause flat spots. A baby’s skull is very soft and pressure from everyday surfaces, such as beds or car seats, can cause flattening.

No two cases of plagiocephaly are alike.
