2019 Year in Review: Care Transitions and post-acute Care 2019 was an exciting year for the care transitions and post-acute care industries. Major changes occured with the new Medicare discharge planning rule and Patient Driven Payment Model going into effect. Additionally, we saw several trends in post-acute care utilization and consumerism.
