Staying connected to care is a powerful step toward feeling better. Follow-up appointments give you space to talk through what’s working, what’s challenging, and what support can help you move forward.
When you show up for care after a hospital or crisis visit, your provider can help you adjust your plan, talk through medications, and connect you with resources that make the next steps easier.
Make sure that you're staying in touch with your provider and letting them know if your contact information changes or if you need to reschedule an appointment.
For more tips on follow-up care and how it could help you achieve your recovery goals, visit: https://cbhphilly.org/members/member-resources/icps/
If you need additional support, CBH Member Services can help you understand your benefits and connect you to care. Call 1-888-545-2600.
