🔥Hot flashes may be present in 75-80% of women in US. Usually start right in the perimenopausal period but sometimes symptoms may start after menopause - defined by no cycles for 1 year without evidence for other pathologic cause. Average age is around 51 years.

🔥Most of the time, hot flashes are related to menopause BUT you also need to consider other conditions such as carcinoid- will be taking about it one of these days⭐️⭐️⭐️

🔥Usually symptoms start in about 40% of women when they are transitioning from peri-menopause to menopause and symptoms increase from 60-80% in late menopause.

🔥🔥🔥Hot flashes are also called VASOMOTOR SYMPTOMS, and tends to be a very common reason why patients would want to seek attention with ob-gyn and/or endocrinologist.

🔥Patients who have an increase risk for getting more hot flashes are:
👉🏻Obese patients
👉🏻Cigarrete use
👉🏻Lack of physical activity
👉🏻Socioeconomic status
👉🏻Ethnicity (African American have a higher frequency than caucasian), Asian population report less frequent hot flashes.
👉🏻Hormonal concentrations - levels of FSH or follicle-stimulating hormone may be associated with increase in frequency.
👉🏻Genetics

🔥Duration❓may last from 4 up to 10 years... 😩😩😩Yep, some studies have pointed out that about 1/3 of patients may experience moderate or severe hot flashes for 10 years after the last menstrual period.

✅Testosterone is a hormone that should be given to men who has clinical symptoms of low testosterone and have confirmatory results proving that testosterone levels are low.
✅There are many symptoms suggestive of low testosterone and t👉🏻he most common ones suggestive of this hormone deficiency is low libido, decrease morning erections, loss of body hair, bone loss, breast enlargement and smaller testicle size.
👉🏻Other more subjective and less specific symptoms include fatigue, depression, decrease in muscle strength and increase in body fat.

✅The treatment is to aimed to restore levels to a normal concentration.
As we get older, testosterone levels decrease, and it is still not clear whether replacement of this hormone is beneficial.
👉🏻If your levels are normal, and there is a suggestion to start testosterone hormone replacement therapy to help with fatigue, consult with a endocrinologist first. Testosterone does have many risks, and therapy needs to be closely supervised.

✅BMI (body mass index), is a very inaccurate measure of body fat content which does not take in consideration muscle mass 💪🏼, bone density🦴, and overall body composition.
As you can see, these women 👩🏻‍🤝‍👩🏼👩🏻‍🤝‍👩🏼👩🏼‍🤝‍👩🏻may look different (taller, shorter, overweight), but yet, they still have the same BMI.
As you can see👀, BMI does not tell the whole story.
✅We need to come with a more practical and affordable tools to measure fat, skeletal muscle. We should be able to determine an ideal weight for a particular individual considering other factors such as sex, genetics, age, pre existing diseases.
✅There is a subset of patients that do have a “normal BMI” and yet have an increase mortality risk , as they are metabolically unhealthy.

✅How to calculate this risk?🤔
You can calculate your 10 year cardiovascular risk of heart disease or stroke using the ASCVD algorithm which was published in 2013 by the American College of Cardiology and American Heart Association ACC/ AHA follow this link : https://www.cvriskcalculator.com

This calculator assumes that you HAVE NOT HAD a prior heart attack or stroke.

➡️Unfortunately, there is insufficient data to reliably predict risk for those less than 40 years of age or greater than 79 years of age and for those with total cholesterol greater than 320.

➡️Optimal age to suggest a first scan is usually earlier in men and in patients who have diabetes.

- Men with diabetes: 35 to 38 years
- Women with diabetes: 49 to 52 years
- Men without diabetes: 41 to 44 years
- Women without diabetes: 56 to 60 years

✅Therapeutic options:
- Scores of 0 - recommendation is against statin therapy for up to five years , so repeat risk stratification should be done UNLESS new other risk factors are present, such as diabetes, active smoking, hypertension, family history of premature coronary artery disease.
- Scores >100 or >75 percentile for age and sex independent of cardiovascular risk should be treated with statin therapy for age and sex independent of the cardiovascular risk.

✅ BMI is a practical way of evaluating degree of excess weight and is based on this calculation: body weight (in Kg) divided in Height (in meters) squared.
✅ BMI classifications are based on risk of cardiovascular disease ❤️ (CVD), which has been adopted by the WHO for different groups (white, hispanic, black, asian)

👉🏻Underweight 18.5 - 24.9 Kg/m2
👉🏻Overweight >25 to 29.9 Kg/m2
👉🏻Obesity >30 Kg/m2
- Class I - 30-34.9 Kg/m2
- Class II -35.0 - 39.0 Kg/m2
- Class III - >40 Kg/m2

👉🏻In South Asian population, the BMI of 21 Kg/m2 is associated with an increase risk for CVD.

👉🏻 One of the BMI flaws or limitations is that may overestimate body fat among patients who are physically fit and muscular 💪🏼 but underestimates body fat in those with lower muscle mass.

📍 For example: A basketball player who is 5’11 and weight is 220 lbs, has a BMI is 30.7 and would be considered to have obesity, when most of this weight comes from body muscle mass 💪🏼and not fat, and is most likely that he may not have health problems related to his weight.

👉🏻 BMI in certain situations may not give has accurate results.

✅ Some studies suggest that waist circumference may be a more reliable predictor of weight related health problems but physicians are not routinely measuring waist circumference.
✅ From recent years, research has evolved in concentrating the efforts in trying to find association with cardiovascular risk and other factors behind the value of BMI or visible subcutaneous adipose tissue, It seems that the “Hidden fat” also called visceral body fat as well as the ectopic adipose tissue (fat located in visceral organs where location is not classically associated with adipose tissue storages such as fat located in muscle 💪🏼, liver, heart ❤️ and pancreas) have a strong correlation with cardiometabolic risk and cardiovascular events.
‼️As Dr. Ian Neeland suggested: "It is time to shift focus from the BMI to visceral and ectopic fat burden” ❣️

✅Thyroid hormone replacement therapy is one of the most common prescribed medication among endocrinologist. It is considered a life long therapy but can this treatment be stopped for some patients? 🤔

✅This study was published in 2021 and has looked at multiple previous published studies where thyroid hormone replacement therapy was discontinued. Authors were trying to identify what would be the predictors of a successful discontinuation of thyroid therapy and how this has been accompllished✨

✅Authors looked at 17 observational studies which included a total amount of 1103 patients, Among them , 86% were women 🚺. About 1/3 of the patients underwent thyroid hormone discontinuation therapy have remained with normal thyroid function test and clinically euthyroid at subsequent follow up. 👍

✅Patients with the diagnosis of overt hypothyroidism (TSH >10 and symptoms of hypothyroidism) were LESS likely to remain with no symptoms and have normal thyroid labs (euthyroid) compared to patients who had a diagnosis of subclinical hypothyroidism (see my prior post).

✅In conclusion , the study indicates that up to a third of patients had remained with normal thyroid test results and no symptoms of thyroid dysfunction after thyroid therapy was stopped. 👉🏻More robust data needs to published to confirm this conclusion as study was somehow limited.

👩🏻‍⚕️In my experience, I have been able to titrate to off patients who have been treated with thyroid hormone replacement therapy, particularly if initial thyroid function test were not that abnormal upon presentation, and patients were relatively asymptomatic; where it is questionable as to why they were started on thyroid replacement therapy to begin with.

👩🏻‍⚕️This should be closely monitored by an endocrinologist as close monitor of symptoms and thyroid function test is needed. It is key to be very selective on the patients that you will be deprescribing thyroid hormone replacement therapy.
