CH PATHway
The PATHway Program (Providing Alternative Transitions to Housing) is an Outreach project of Chicago House and Social Service Agency.
PATHway serves homeless and chronically homeless adults (ages 18 and older) who are living with chronic medical conditions and in need of assistance transitioning from homelessness to stable permanent or supportive housing. The Pathway project target population are individuals impacted by poverty, homelessness and chronic Illnesses.
