Do you have a little one starting school this fall? Mark your calendar for Thursday, April 23rd and head to your neighborhood elementary school between 9AM and 3PM to register.
Get a free developmental screening for your child by contacting us prior to your visit for school registration.
Don't forget what to bring for registration: your child's birth certificate, a parent/guardian photo ID, proof of residency, and a school health entrance form.

We are excited to announce our 2nd Programs with Heart Mother's Day mental health support group.
This workshop will provide mental health support to parents enrolled in Child Health Partnership programs. This in person group will be a relaxing creative workshop where parents come together to make a simple craft, connect with others and enjoy a moment of creativity.
