Yesterday was the first day Michael K Ryan, MD and his team started seeing new patients in Pell City. The sub-specialty clinic is located within Pell City Internal & Family Medicine, 41 Eminence Way, Ste. A, Pell City.

Thank you to the Pell City Chamber of Commerce for the warm welcome.

Dr. Ryan specializes in surgical sports medicine, hip preservation, hip replacement (anterior approach), knee and shoulder.

🤷‍♀️🤔 What’s Wrong Wednesday- Follow Up 🤔🤷‍♂️

❓Did you answer correctly⁉️ 🙋

🧐 Diagnosis ✍️…This patient sustained a glenohumeral joint (shoulder) dislocation where the ball pops out of the socket ☄️

✅ As the ball 🔘 is forced over the edge of the socket, the labrum and associated ligaments are torn 🔥 or damaged (called a Bankart tear), which can result in recurrent instability (shoulder continues
to slip out of place)

✅ Occasionally, the dislocation breaks of a piece of the front of the edge of the socket, known as a bony 🦴 Bankart

✅ In addition this patient also tore his rotator cuff, the tendon that attaches at the top of the ball to help lift the arm overhead 🏋️‍♀️

✅ Individually, each type of injury frequently requires, but the combination of both injuries necessitates surgery to allow for proper healing 🙏🏻 and restoration of function 🙌

🧘‍♀️✨

🌟Meditation was a foreign concept to me until about 5 years ago

I tried it after realizing many highly successful people in highly demanding careers found it incredibly useful to:
▪️ Gain a new perspective
▪️ Increase self awareness
▪️ Focus on the present
▪️ Harness the irrational internal voice
▪️ Employ rational thought over emotion

I started meditation with Headspace, a simple, effective app that teaches the basics of breath work 😮‍💨 and mindfulness 🧖.

I have also used the Waking Up app by Sam Harris which is a deeper dive into the mind and consciousness. Both are excellent methods to learn to calm the mind.

💫 If you’re still skeptical, check out the book and app 10% Happier (Ten Percent Happier) by Dan Harris (no relation), which is a great example of how meditation does not “make you soft” or remove your edge, but instead allows for a more lucid mind 🙏🏻 to attack the day❕

🤕 Today is World Mental Health Day.

Historically, mental health has been stigmatized and thus kept quiet. 🤫

Recently, the pervasiveness of mental health has become apparent on a global 🌎 level. Athletic giants like Michael Phelps, Simone Biles, Naomi Osaka, and many others have bravely crossed the boundary of vulnerability to demonstrate the reality and importance of mental health.

As a physician, whose profession is rife with stress as evidenced by high burnout rates 📈 and su***de rates twice that of the standard population, it has never been more apparent how important mental health is.

A few months ago clinical & sports psychologist, Dr. Bhrett McCabe, joined me on the Victory Over Injury Podcast. His words were so powerful and enlightening. Check out the link below to hear the highlights on mental health from that episode.

🧐 Diagnosis ✍️…This patient sustained a distal humerus fracture 💪 🔥

✅ Arm wrestling places a large torque on the humerus as forced internal rotation is resisted 🤝 and opposed
✅ In this case the torque was greater than the bone 🦴 could resist, causing a spiral fracture 💥 of the distal humerus
✅ With displacement and angulation in an young active individual, surgical fixation was indicated
✅ This fracture was fixed with two plates ⚪️⚪️ and several screws 🔩🔩
✅ After healing, the patient returned back to full function and activity 💯🙌🙌

📣 Check out one of my favorite 🙌 exam maneuvers for the hip 🦴

✅ Labral tears 💥 are common in the young active population with hip and groin🦵pain
✅ Labral tears result from bony impingement (FAI), dysplasia, or other bony 🦴 abnormalities
✅ To test for a labral tear, put the hip in a position of impingement by Flexing, ADducting, and
Internally Rotating (FADIR) the hip
➡️ A modification of this is more of a “scour” where the femoral head is rotated along the front arc of the socket
➡️ A ➕ positive test induces pain 🔥 that is consistent with the same pain the patient feels with activity

🤷‍♀️🤔 What’s Wrong Wednesday- Follow Up 🤔🤷‍♂️

❓Did you answer correctly⁉️ 🙋

🧐 Diagnosis ✍️…This patient suffered a lateral meniscus tear 🦵🔥

✅ The unique part about this tear is that it was flipped and extruded (pushed out) laterally into the lateral gutter, causing pain 💥
✅ After freeing it, it was apparent that the large amount of meniscus tissue was more than is
normally seen, indicative of a discoid-type meniscus
✅ Due to the complex nature of the tear 🔥, and the discoid-type tissue, a partial meniscectomy was
performed, as suture repair would have failed due to inadequate blood supply to the central portion of the meniscus
✅ After the partial meniscectomy, a notable amount of normal meniscus tissue remained
✅ Once the patient regained his full quad 🏃‍♂️ strength he was cleared to return to football 🏈 🙌🙌🙌

🏈 Football season. Football Injuries.

🧐 The details…✍️

✅ 14-year-old male linebacker was rolled up on while making a tackle
✅ He was hit on the outside of his left knee 🦵 with a valgus force and felt immediate pain 💥
✅. He was able to walk 🚶‍♂️but unable 🚫 to return to play due to pain 🔥 over the lateral (outside) aspect of the knee
✅ Range of motion was limited in extension, and he noted ✍️ catching when trying to bend

📣 This is called the Resisted Supination External Rotation Test (RSERT)...
➡️ Originally described by Myers TH, et al: Am J Sports Med. 2005;33(9):1315–20...

✅ This is one of the 3 SLAP 👋 tests typically utilized for the overhead 🙆 athlete who is exhibiting signs of a SLAP Lesion…

✅ Place the arm at 90 degrees of abduction & external rotation with forearm 💪 pronation - now resist supination, external rotation & elbow flexion… positive test would be reproduction of symptoms in the superior shoulder area!

⚡️⚡️Try this test - I think you'll find it beneficial.

🧐 Diagnosis ✍️…This patient experienced a full-thickness injury of the cartilage on the weight-bearing surface of the medial femoral condyle

✅ The impact of the injury and the subsequent loss of cartilage results in bony edema (swelling)
and pain 🔥
✅ This injury may also cause recurrent effusions in the knee 🦵 joint in addition to pain 💥
✅ Non-surgical attempts may be attempted, including PT and injections, but often fail to address
the source of pain
✅ Several options are available for cartilage preservation in the knee and other joints 🦵, including microfracture, osteochondral allo/autograft transplantation (OATS), and autologous chondrocyte transplantation (ACI/MACI), and each option has a specific indication
➡️ Larger lesions ~20mm, such as this one, benefit from OATS, and possibly ACI/MACI for large
lesions
➡️ The large size, location, and bony edema made this lesion best suited for an osteochondral
allograft transplantation (OATS) from a cadaver
✅ Techniques have allowed for optimal graft incorporation, healing, and recovery
✅ This patient was able to return to a high level of pain-free 🚫 function 🙌🕺🎾

✅ He doesn’t recall any specific injury, but while playing collegiate tennis remembers tweaking
the knee several times
✅ He denies any instability, catching, or locking, but reports significant swelling 🔥 in the knee after impact exercise or playing tennis 👟🎾
✅ The swelling goes down ⬇️ over a few days of rest

🤷‍♀️🤔 What’s Wrong Wednesday- Follow Up 🤔🤷‍♂️

❓Did you answer correctly⁉️ 🙋

🧐 Diagnosis ✍️…This patient has external snapping of the hip (coxa saltans)

✅ Snapping of the hip 🦵 is classified as internal, intra-articular, or external
✅ Internal snapping 💥 can be heard 👂, while external snapping can be seen 👀
✅ External snapping occurs when the iliotibial band forcefully snaps over the greater trochanter
causing a visible snap
✅ The iliotibial band (IT band) is a confluence of connective tissue (fascia) from the tensor fascia lata (TFL) and gluteus maximus muscles
👉 Most patients with external snapping 💥🦴 hip improve with non-surgical treatment, but this patient's case was severe, significantly limited her ability to adduct her legs, and limited her function
👉 When non-surgical treatment fails, a z-plasty of the IT band can be performed to release tension on the IT band, thereby eliminating 🙅‍♀️ snapping

🧐 The details…✍️

✅ 16 year old female runner 🏃‍♀️ who experienced lateral hip pain 🦵 for years
✅ Noted pain 💥 and a mechanical clunk over the outside of her hip with certain activities as well as
difficulty crossing ❌ her legs
🚫 Prior injections, PT, NSAIDs, stretching all failed

🤷‍♀️🤔 What’s Wrong Wednesday- Follow Up 🤔🤷‍♂️

❓Did you answer correctly⁉️ 🙋

🧐 Diagnosis ✍️…This patient sustained a spiral fracture of her proximal femur 😳

✅ The femur (thighbone) 🦴 is one of the strongest bones in the body, and typically only breaks in
high-energy injuries (car accidents, falls from heights…)
✅ However, in older populations (>60-70), fractures of the proximal femur (closer to the hip) are more common with low-energy falls
✅ Hip fractures🦵are classified into different types (femoral neck, intertrochanteric, subtrochanteric) based on the fracture location
✅ The location, type of fracture, and degree of displacement dictate the best treatment options
✅ This fracture is slightly atypical as it occurred in the proximal shaft with a low energy fall, and
approaches the subtrochanteric region
✅ Surgery is necessary to fix this fracture to allow for prompt mobilization, patient comfort,
decreased 📉 pain 💥, bleeding, and swelling
✅ With an intramedullary nail, patients may weight bear immediately after surgery 🙌🙌
✅ Healing was seen at six weeks, and this patient was able to return to normal function without
any assistive walking devices 💯

💫 Lichtman was a four-year scholarship athlete on the Stanford University women's volleyball 🏐 team, and was selected All-American First Team in 2009 and 2010, and First-Team Academic All-American in 2010. She competed on the U.S. Women's National 🇺🇸 Volleyball team from 2011 to 2016, making every roster in 2013 on her way to numerous international medals 🏅🏆.

Cassidy took on coaching in 2016 as an assistant at Stanford and was part of the National Championship team that year. She founded “Play with Purpose” volleyball clinic, which supports the Ronald McDonald House, and recently launched P/ATH (Progress through Athletics) a platform that seeks to position the next generation of athletes for a better world through empathy and empowerment❣️

🧐 Diagnosis ✍️…This patient sustained an acute, full-thickness tear 💥 of her rotator cuff

✅ The rotator cuff is a group of four muscles 💪 and their tendons that attach on the top of the
humerus (arm bone) 🦴
✅ The rotator cuff facilitates smooth and easy motion of the arm, particularly lifting it overhead 🙆‍♀️
✅ The supraspinatus tendon is the most commonly torn tendon
When torn patients have pain 💥 with overhead activities, reaching across or behind their body,
and with sleep 😴; weakness can be present as well with bigger tears 🔥
✅ Arthroscopic surgery is recommended for full-thickness, acute tears
