One of my favorite areas to treat to provide q-ties with a more “refreshed” look is the midface. The midface consists of the lower eyelids and cheeks. The top concerns that patients have regarding this area include the following:

👀Lower eyelid wrinkles
👀 Dark circles under the eyes
👀 Puffy lower eyelids
👀 A sunken in area in the lower eyelids (tear trough deformity)
🧏🏽‍♀️ Flat cheeks
🧏🏽‍♀️ Laxity and descent of cheek skin
🧏🏽‍♀️ Prominent nasol***al fold lines (“parentheses” lines)

This gorgeous q-tie was bothered by the puffiness of her lower eyelids and felt that she often looked tired. She came in to discuss tear trough filler but, because of the excess lower eyelid fat, I felt that she wasn’t an ideal candidate for tear trough filler. But, she insisted on having something done to help her look less tired. I recommended cheek filler to help mask the under eye puffiness and restore volume to her midface. Some of us never had full cheeks. But, with age, even those of us who always had full cheeks will see that volume decrease and the skin in that area begin to sag. Cheek filler is an instant rejuvenator… it even makes young people look younger. In this q-tie one syringe of Voluma was used in each cheek (2 syringes in total) to augment the volume of her cheeks.

What is a l***aplasty?
Labiaplasty is a surgical procedure performed on the l***a minora (inner vaginal lips) to reduce their size.

Who is a candidate for a l***aplasty?
Some women have significant redundancy of the l***a minora tissue, making the l***a minora bulky. Other women have l***a minora that hang significantly below the l***a majora (outer vaginal lips). Sometimes, this anatomy is the result of genetics. In other cases, the anatomy was altered when the tissues were stretched during child birth.

Why have a l***aplasty?
Many patients are very self-conscious about the appearance of their l***a. In addition, patients with bulky or hanging l***a minora may experience pinching of the tissue in certain clothing or when participating in exercises such as bike riding. Sexual in*******se can be painful for the same reason. Also, there may be hygiene issues related to the redundant tissue.

Lips are so varied and there’s a wide range of what constitutes “normal”, which is why lip filler must be customized for each person’s unique features... AND goals. Here are 5 reasons why I love lip fillers:

👄 Lip fillers are great for adding volume to lips, but they do so much more than just that.

👄 Lip fillers can smooth out fine lines and wrinkles along the border of the lips and within the lips to create, smooth, full, hydrated lips.

👄 Lip fillers can be used to define/enhance certain anatomic landmarks. (Hello Cupid’s bow! 💘)

👄 Lip fillers are great for balancing out lip proportions. For example, some patients have an upper lip that is disproportionately smaller than the lower lip.

Do you know what a breast implant capsule is?

Once breast implants are inside of your body, your body forms a wall (capsule) around the implant. This is a normal process and happens with anything artificial in your body- a pacemaker, a prosthetic knee, etc. Breast implants don’t have a set expiration date but that doesn’t mean that they will last forever. As breast implants age, the capsule around them can become thick, hard (calcified), or distorted. This is a condition known as capsular contracture. When this happens, the capsule may begin to constrict the implant, resulting in a misshapen breast. Capsular contracture can be a natural consequence of the implant aging or can occur when the implant has ruptured. Symptoms of capsular contracture include breast pain, a firm breast, and/or distortion of the implant. The treatment for capsular contracture is removal of the implant and capsule.

Are you trying to figure out if Botox and filler will be enough to rejuvenate your eyes or if it’s time for eyelid surgery? Here are 5 things that may help you with your decision:

👁 Crow’s feet lines (lines in the lateral corners of the eyes that are more pronounced with squinting or smiling) can usually be improved with Botox.

👁 A sunken groove (tear trough) at the inferior border of the lower eyelid along the eyelid/cheek junction can be be due to loss of volume in that area and traction of ligaments due to descent of tissue with age/gravity. Fillers may be used in the tear trough area and/or cheek to correct this and refresh the lower eyelids.

👁 Dark circles are very challenging to treat and treatment varies with the cause. Eye creams, lasers, microneedling, fillers, or a combination of these can be used to treat dark circles. However, the results are less predictable than when these same modalities are used to treat wrinkles, skin laxity, or volume loss.

👁 Mild lower eyelid skin laxity can be addressed with microneedling, lasers, and radiofrequency treatments, among other things. However, significant amounts of redundant skin will need to be surgically removed.

👁 Significant amounts of excess eyelid fat resulting in “bags” or “puffiness” are best addressed with surgery.

Skincare products containing salicylic acid may help, especially if you have oily skin. Salicylic acid is a beta-hydroxy acid (BHA) that is fat soluble and, thus, able to pe*****te deep into pores. It is a go-to chemical for fighting bacteria and treating acne in people with oily skin. It can be found in a variety of products- facial cleansers, toners, serums, spot treatments, acne dots, etc. So, if you’re experiencing maskne, consider adding a salicylic acid product to your skincare regimen.

Dr. Aisha White is a board certified plastic surgeon specializing in breast surgery (including breast augmentation, breast lift, and breast reduction) and cosmetic surgery of the face and body. She attended the Feinberg School of Medicine at Northwestern University and concurrently completed a Master of Business Administration program at the Wharton School of the University of Pennsylvania.
