🚨 IMPORTANT TELEHEALTH UPDATE FOR RHCs & FQHCs 🚨
CMS has released an important update regarding billing for distant-site telehealth services, with changes taking effect October 1, 2026.
Beginning October 1, RHCs and FQHCs will be required to bill the individual CPT or HCPCS code that describes the distant-site telehealth service provided instead of HCPCS code G2025. The appropriate revenue code and telehealth modifier (93 or 95) will also need to be reported.

CMS Plans to Replace G2025 with HCPCS Billing for Medicare Telehealth in October 2026

The Centers for Medicare & Medicaid Services (CMS) recently released Change Request 14468, instructing Medicare Administrative Contractors (MACs) to modify the billing system through which RHCs bill for Medicare distant site telehealth services.
Currently, RHCs bill G2025 for any of the 280+ services performed via telehealth, which reimburses one single rate, $97.53 in 2026. While this has allowed RHCs to serve as distant site providers since 2020, it has imposed significant barriers on safety-net providers seeking to offer telehealth services to their patients.
As the RHC community knows, The National Association of Rural Health Clinics has advocated both legislatively for a fix to the special payment rule, which pays significantly less for telehealth services than an in-person encounter, and through regulatory pathways, encouraging CMS to fix the special coding rule that obscures and distorts claims data by simply using one code. Through this change request, CMS is now fixing this coding issue.
Specifically, effective October 1, 2026, RHCs shall bill the individual HCPCS codes describing the telehealth services furnished to Medicare beneficiaries, instead of G2025 for every service provided. Additionally, RHCs will be required to report modifier 93 for audio-only services, and modifier 95 for audio-visual telehealth.
