Indonesia Launches Search for Missing Submarine

Sad to read about the Indonesian Navy submarine KRI Nanggala which went missing during a drill off Bali early on Wednesday. This is per the Indonesia's defense ministry's report in several maritime news reports. As a submariner I am always saddened at such reports. I think, India may be the closest nation with the DSRV ability and has hopefully offered the help. The reaction time has to be minimum. Considering that a sleek of oil was seen, and depth of bottom is approximately 200 meters more than the maximum depth to which this 44 year old sub could dive, it seems like a tragedy in the making. As an ex or more appropriately old submariner, I can only pray for a miracle.

[03/30/21]   QMII covers soft skills, to further strengthen the management systems (MS). MSs when well implemented and used aggressively, can stand an organization in good stead in a crisis situation. Crisis management should be an extension of the well implemented organizational machinery functioning as a well oiled machine. Risk in the context of the changed context (the crisis) should be the determent for decision making. Standard crisis and risk management preparation based on videos, clips and scenarios without connection to the organizational MS could be counterproductive. QMII specializes in leadership training, conflict management, implementing management systems and the role leadership plays and is therefore aware of the correct pattern of training, its entertainment value, and certainly some amount of practice for a potential crisis. Each crisis in unique in its own way. Therefore, crisis and risk management response also are not a fixed pattern based on just drills. Drills, practice and exercises are vital, but response to crisis without connecting the facts to the management system will not give the confidence that the response can be supported by the system . In Crisis often the guidance is to communicate to keep the organization protected. To present the truth but on a need-to-know basis. QMII assists leadership, to design training as a flow from the management system of the organization. This will then teach participants to communicate basis the confidence that the organization can meet the expectations as communicated.

Crisis management and crisis communication are related and yet the communication should not make management response a difficult task. Projecting efficiency and predicted preventative actions should have a connection to the organization’s MS. Crisis and risk management is the MS working more efficiently. Not necessarily at variance to the MS. Crisis communications should convey this system approach of calm calculated planned response and a team doing what they should.

QMII has in 30 plus years, since 1986 assisted organizations, in putting in place a MS that works. This should be the underlying principle for crisis management and crisis communication. With our experience and expertise we can produce more relevant material, videos and training content which is company specific and considers the context of the organization.

Suez! I have transited the Suez Canal several times, as also the Panama and so on. Suez is a unique waterway. The pilots and Captains must work together to ensure shallow water affect is considered when navigating the canal. This is particularly important at wheel over positions/turning points where the stern of the ship may be closer to the shallow bank. Shallow water can cause the stern to squat and run the ship aground. The pilots are experienced and usually manage it well. Larger ships, as in this case, with a fully loaded cargo can sit deeper. For tankers it is just that meaning they sit deeper but container ships also have the wind to take care of. Ever Given is a large container ship, as large as she is, the wind sale area is prone to drift by the winds, in this case the sandstorm. Of course, the pilot with the vessel power manages to control it in the normal circumstances. However, if the main engines fail even for a short while, the wind area becomes prominent to push the ship to lie beam on to the prevailing wind. Broadsides to the wind is never a comfortable feeling. Probably that is what happened. Just my hypothesis. The inquiry will tell, I guess. Swiss cheese cake model!!! One day/ some day all holes line up!

Title: AS9100 Aerospace Industry Requirements

ISO9001 meets all the generic requirements of the various industries, by suitable interpretations by the organization when implementing the system, auditors when auditing and trainers by providing the requisite training in academic understanding and purport of the standard. However, certain specific industry requirements need a more aerospace specific interpretation and the incorporation of unique industry specific terminology, interpretation, and accepted norms. The standard must still be interpreted, but some of the clauses have a prescriptive and descriptive element. The AS9100 does just that.

The aerospace element not only requires aerospace maintenance AS9110, reliable supplier and providers AS9120 and the generic aerospace standard AS9100. The need is to assure customer satisfaction across the industry needs for aviation, space and defense organizations. The products provided must be safe and reliable and the services related to aerospace must meet not only customer expectations but essentially all of statutory and regulatory requirements.

This would be a challenge for any global industry, but particularly the aerospace industry. AS9100 when correctly implemented provides the management system framework in meeting the global aerospace supply chain diversity requirements. These requirements of AS9100 are not just for the organization, but for the domestic national requirements as also of the various countries and their expectations at various levels of the supply chain. The organization, by its global outreach will require to outsource what it does not do or what is required at various locations connected by the planned aerospace supply chain. All that it means that the organization can review the applicability of the requirements based on its size, complexity and decide on the activities it needs to outsource meeting the industry requirements as interpreted from AS9100 and or any of the related standards.

The AS9100 therefore is now not an ISO standard, even though it incorporates the ISO9001 and builds on it. The industry has established the IAQG – International Aerospace Quality Group which has representatives from aviation, space, and defense companies from all over the world to pool the knowledge, experiences, lessons learnt, resources and talent to come up with clauses which when implemented to create the management system for the organization based on AS9100 or any of the variants gives the best chance of universally acceptable quality in aerospace functioning.

No standard can guarantee the outcome. AS9100 too cannot warranty the outcomes but can give the organization the best chance to initiate the processes which will provide confirming products and services and ensure safety for the aerospace supply chain. With diligent motivated participatory employees, management, and clientage AS9100 provides the framework for continual improvement. In terms of auditing, it is incumbent upon auditors that they carefully seek conformity to requirements of AS9100 when auditing. Internal audits to AS9100 must provide the objective inputs to top management to make better decisions and appreciate the risks of their products and or subproducts and services, as also when outsourcing to outside organizations, remembering they remain accountable and answerable for the final product or service. Ensuring the organization accountability for the conforming products and services whether outsourced or not is the responsibility of the organization. AS9100 framework, as conceived based on clauses from ISO9001 and the IAQG requirements with added clauses is the start of an efficient safe aerospace organization working efficiently.
