History: A 60-year-old woman with atrial fibrillation underwent a percutaneous left atrial radiofrequency ablation under general anesthesia. Transesophageal echocardiogram confirmed no thrombus prior to ablation. The procedure was complicated by right atrial rupture causing tamponade physiology. Her anticoagulation was reversed with protamine, and a pericardial drain was placed. She was extubated 6 hours after induction and found to have a left hemispheric syndrome with dense right hemiparesis, left gaze preference, and global aphasia (NIHSS 27). She underwent percutaneous embolectomy. Her 30-day modified Rankin Scale score was 3, and she was living at home with her spouse.
