🇬🇧 « Deep Margin Elevation »(DME) or « how to switch from a difficult situation to a simple one ».
DME is suppose to be difficult, but sometimes, little tricks offers a lot of serenity.
Double matrix gives you stiffness, and allows you to jab in between tooth and rubber dam.
Matrix is then pressed by the rubber dam on the cervical margin with a perfect seal,
without additional wooden wedge of Teflon tape.
When it’s done, it becomes an easy case !

🇫🇷Remontée de marge (DME) ou comment passer d’une situation compliquée à un cas simple.
La DME a la réputation d’être difficile à mettre en œuvre correctement.
Un petit truc qui peut apporter beaucoup de sérénité consiste à utiliser 2 matrices, ce qui leur confère une bonne rigidité, pour les planter entre la digue et la limite cervicale profonde. La matrice est alors parfaitement plaquée entre le digue et la limite cervicale, assurant une bonne étanchéité, sans coin additionnel ou Téflon. Quand cette étape est terminée, les difficultés sont levées 🍾!

I don’t know what is your feeling about it, but personally, I feel I encounter cracks everywhere under amalgam restorations.
How deal with these enamel or dentin cracks ?
Bio mechanical structural analysis give some insights to choose the less risky restoration.
Here, 2 disilicate overlays were done. What about you ?

Je ne sais pas pour vous, mais personnellement j’ai l’impression de trouver des cracks partout sous les amalgames.
Comment gérer ces fêlures, amélaires ou dentinaires ?
L’analyse biomechanique des structures résiduelles permet de correctement choisir le type de restauration le moins à risque.
Ici, deux overlays en dicilicate. Et vois qu’auriez vous fait ?
