አማኑኤል ተኝቶ ከመታከም በፊት የሚደረገው ውል

አማኑኤል ሆስፒታል ተኝቶ ለመታከም ከሀኪም ውሳኔ በተጨማሪ ታካሚው ወይም ቤተሰቡ መስማማትና ከሆስፒታሉ ጋር የውል ስምምነት ማድረግ አለበት። ውሉ የሚከተለው ነው።

1) በሀኪም ይተኛ ተብሎ ሲወሰን አጠገቡ ሆኜ እንዳስታምም አስፈላጊውን ሁሉ ለማድረግ ፈቃደኛ መሆኔን።
2) በሆስፒታሉ ቆይታ ወይም ሀኪም ወስኖ ይውጣ ሲል ለማስወጣት/ለመውሰድ ተስማምቻለሁ።
3) በሀኪም ወጪ ተብሎ ሳላወጣ ብቀርና ሆስፒታሉ ወጪ (Discharge) በተባለበት ሰአት ታካሚውን ለመውሰድ ሳልገኝ ብቀር ቅድሚያ ባስያዝኩት ብር ሆስፒታሉ ታማሚውን ወደመጣበት አሳፍሮ እንዲልከው ተስማምቻለሁ።
4) በሆስፒታል ውስጥ ከአቅም በላይ የሆነ ችግር እንኳን ቢያጋጥምዎት የሚከታተሉትን ሀኪሞችና ነርሶች ማነጋገር ያስፈልጋል።
5) በሆስፒታል ውስጥ ታካሚውን እንዲታከም አስገብቼ ጥዬ ብጠፋና ሆስፒታሉ ወደ መጣበት አሳፍሮ ሲልከው ቢጠፋ ሆስፒታሉ ተጠያቂ አይሆንም።
