In my review of the evidence for the upcoming blood pressure meds Co-Prescriber LIVE webinar on Tuesday night, I read this paper.
But it’s not particular to the presentation, so I wont be discussing it - even though it is EXCELLENT!

⁉️What are your go-to recommendations for arterial calcification?
For me, lots of dietary interventions, fish oil, K2 etc
But if I’m honest - garlic was not always on my list… if hypertension wasn’t an issue I didn’t prioritise it
This paper will change that for me and so I want to tell every co-prescriber!

What is your go-to reference for drug-herb interactions?
I have a few resources myself - nutritional medicines database is my first stop (requires an annual subscription, I purchase mine through )
Being fully referenced you can see a full list of potential drug-herb interactions, and the evidence to support.
Drawback is that it is overly cautious - when you investigate the interactions they are often not specific to that herb or drug.
So it takes a bit of time.
I also like the interactions chart at the back of Hechtmans naturopathic text ‘clinical naturopathic medicine’
I also use IM gateway and eMIMs
All this reference checking takes time though - which is why one of the lessons in each of the modules for the Co-Prescriber is dedicated to summarising all these sources
For these most common meds, I provide you with a central and succinct summary for the safe use of herbal medicine with these drugs.

What a whirlwind the past two days has been at the Pharmacy Forum!
I have always loved conferences, whether attending as a participant, an exhibitor or even a presenter 🥰 and the Forum is always such a great example of why.
It’s the infectious energy and excitement of 500+ likeminded health professionals getting together to connect, collaborate and catch up.
Networks are strengthened, innovations are explored and plans are made - all jam packed into two fast paced days.
I am always honoured and so proud to be asked to present in the clinical stream. Shining the spotlight on evidence based complementary medicine is my passion and the opportunity to share that with WA’s best pharmacists is just such a privilege!
And to top it all off, this year I was on the big stage, no pressure 😳😂💪

🔔interesting paper to share🔔
In researching my upcoming presentation to at the pharmacy forum next week, I came across this paper about how lack of sleep impacts cholesterol.
Then last week, a pharmacist was asking about the ‘why’ with statin prescribing
And then yesterday, menopause and cholesterol was a topic of conversation in one of my naturopath chat groups so I shared this in explaining the benefit if good sleep.
Which brings me to today, phew!
Ahead of next week I thought to share this little snippet - I see high LDL and poor sleep in clinic ALL the time and it is only recently (after this research!) that I’ve honed in on making these changes for patients and the outcomes have been wonderful.
Essentially, when you don’t get enough sleep, the production of CYP7A1 (enzyme for converting cholesterol to bile acids) is significantly inhibited.
Meaning that cholesterol continues to be produced - but conversion is essentially halted, leaving blood levels to rise 😱

Have you come across the term Patient oriented evidence that matters, or POEMs?
I recently came across this review and I thought it would be useful to share.
Co-Prescribers will be familiar with many of these outcomes but more evidence to confirm our approach is always exciting!
Of the 20 in this review - I see 6 outcomes that you can use with your patients right now
1️⃣ Mediterranean diet being better than 7 other for CVD - such an easy one to recommend with so many good resources available to support your patients
2️⃣ placebo as good as opioids for acute neck or lower back pain… and if your completed the module on these drugs you’ll also have a slew of comp med option to use instead 👏
3️⃣ 70-90 minutes of continuous exercise better than 3x 20-30 weekly sessions for osteoarthritis - an interesting option for those who struggle to commit to standard regimes
4️⃣ ‘healthy behaviours’ lower risk of diabetes… telling us what we already know, confirmation is always good though!
5️⃣ older diabetics with HbA1C over 9% have an increased risk of dementia - I heard dementia referred to as ‘type 3 diabetes’ years ago, all the more motivation to manage BSL
6️⃣ who has done the SSRI module? Another study to show mindfulness being as effective as escitalopram for anxiety! Hurray! Relaxed without the side effects!

My biggest takeaway is the term ‘Patient Oriented Evidence that Matters’! for us clinician, these are the pearls we are always looking for to practically help improve the health outcomes for our patients 💪👏

I’m sitting at the airport, reflecting on the .est1920 conference for 2024 and choosing 10 photos to summarise was difficult as there was just so much to choose from
From .marccohen and his opening address talking about the benefits and history of oxymels, and honey to an afternoon of learning how to percolate plant medicines and a presentation from that challenged my thinking on the use of vitex in men - day 1 had it all
And that’s not to mention the speed dating sessions with who I always love to hear that the basics still have immense value
and I think I signed myself up to the special interest group on sustainability with NHAA that I very much look forward to sharing with you in the future.
My interest in this only grew on day two when listening to and Ian Breakspear on the benefits of both heroic and drop dosing followed by an interesting presentation on growing herbal medicines with
The grand finale was of course the godfather of our industry - an interview with prof Kerry Bone, pics of the goat shed that started mediherb was a highlight.
And then the catch up with colleagues and friends on the exhibitor stands is always fun and such a great chance to learn about and taste test the latest and greatest for new supplements out there
So fun, so fulfilling - going home with new ideas and of course some new textbooks on their way 😂

Did you see the CMA response to reports of liver L/GI issues with Withania (ashwagandha)
I have cut and pasted it in full for you to read on slides 2 and 3
I’m posting about it here for a couple reasons.
1️⃣ self prescribing
Withania has been used safely for centuries as part of Ayurvedic medicine and in more recent decades it has become part of the ‘stress & anxiety toolbox’ for many practitioners here in Australia.
In the last couple years though, it seems to have had an explosion across the internet and many are now taking it self prescribed.
Currently there is no warning on the label but the 2023 literature review (pmid 3775601) warns against use in those with pre-existing liver disease.
This is why we as practitioners need to be prescribing herbs like withania -ensuring not only that they are effective but also that they are safe.
2️⃣ Australian Registered products
ALWAYS LOOK FOR THE AUSTL (or R)
This is so important - to ensure that what is on the label, is what’s in the bottle, at the right dose, free from contamination or adulteration.
I have patients who buy online and I always take the time to explain that for quality, efficacy and safety - I need them taking products they are registered on the ARTG as AUSTL or AUSTR
Withania is a wonderful herb, a staple in my clinic - I would hate to see it unnecessarily restricted or removed from our dispensaries.

🌍 The World Health Organisation (WHO) Drafting New 10-Year Traditional Medicine Strategy
📝 WHO is drafting a new 10-year strategy for traditional medicine, set to be presented to the World Health Assembly in 2025. Stakeholders in traditional, complementary, and integrative healthcare (TCIH) have an opportunity to contribute insights.
🔍 Key Findings:
Research Gap: Despite thousands of TCIH-related clinical trials, there are still data gaps, with only 12 countries investing in TCIH research. WHO should promote investment and offer research guidance.
Inclusion in Guidelines: WHO and countries should incorporate safe and effective TCIH options into prevention and treatment guidelines, leveraging evidence from various fields including integrative oncology.
Regulation Needed: Regulation of TCIH practitioners, practices, and products is essential for accountability and trust. Only 78 out of 170 countries acknowledge national TCIH use, despite available WHO benchmark documents.
Product Regulation: Regulation has primarily focused on herbal medicines. WHO should assist countries in establishing risk-based regulations for all TCIH products.
Reorienting Healthcare: TCIH plays a crucial role in shifting healthcare systems towards a person-centered model. The WHO Western Pacific Region’s integration of TCIH provides a model for other member states.
