By now you've probably heard about yesterday's report from the Associated Press, indicating that there is little research-based evidence demonstrating the efficacy of flossing. The federal government has recommended flossing since 1979, first in a Surgeon General’s report and then in the U.S. Departments of Agriculture and Health and Human Services Dietary Guidelines for Americans. Under U.S. law, federal guidelines must be based on scientific evidence. The government has acknowledged that, because the effectiveness of flossing had not been adequately researched as required, the recommendation to floss was removed in the 2015-2020 Dietary Guidelines, which were released earlier this year.

In light of this news, flossing has been a hot topic in the national media. Public sentiment ranges from humorous absolution of guilt (likely the 27% who admitted to lying to their dentists about flossing in a survey we administered last year) to commitment to daily flossing. As periodontists, we treat patients every day for moderate to severe periodontal disease. We witness the long-term value of patients taking care of their gums and teeth by brushing twice a day, flossing regularly, and seeing us every year for a comprehensive periodontal evaluation. For us, the benefits of flossing are self-evident, and we share this message with our patients. We know the havoc that plaque bacteria can wreak on gums and supporting structures of the mouth.

Because periodontal disease progresses slowly, an ideal investigation of flossing’s impact on periodontal health would need to span 10 to 20 years and include thousands of participants, including those with known periodontal disease risk factors such as family history, smoking, and diabetes. A study like this would likely require significant funding.

In the absence of quality data, the American Academy of Periodontology continues to recommend the low-risk, low-cost inclusion of flossing in the public’s daily oral hygiene routine. Yesterday, I had the opportunity to convey this message to some of the nation’s most high-profile news outlets, including The New York Times, NBC Nightly News with Lester Holt, The Today Show, and in a number of radio interviews broadcast across the country. I also appeared on Canada’s CTV News.

The Academy has also released a statement regarding our stance in this flossing debate.

Some of you have already received interview requests from media outlets in your communities. I would encourage all of you to reach out to local newspapers, TV, and radio stations and identify yourselves as an ideal spokesperson on this topic—an expert in periodontal disease prevention and treatment. This attention to flossing provides a perfect opportunity for us to shine a light on our specialty and expertise. The Academy has developed a set of speaking points that you can use to guide your conversations not only with reporters, but with your patients.
